As a key member of our care team, the LPN will provide direct care to residents and supervise nursing personnel to ensure the delivery of high-quality care.
Key Responsibilities:- Supervise nursing personnel to deliver nursing care and coordinate care delivery to ensure resident needs are met.
- Provide direct care to residents, including administering medications, performing treatments, and providing wound care.
- Conduct regular assessments of residents' physical and emotional needs, and develop and implement individualized care plans.
- Collaborate with other healthcare professionals to ensure comprehensive care and address any concerns or issues that may arise.
- Maintain accurate and up-to-date records of resident care, including progress notes, medication administration records, and other relevant documentation.
- 2-3 years of clinical experience, with a focus on gerontological nursing and dementia care.
- Valid and current LPN/LVN state license.
- Ability to read, write, and speak English in an understandable manner.
- Ability to sit, stand, walk, bend, and squat for prolonged periods, as well as lift and carry up to 20 pounds unassisted.
- No mandate to stay over for shift coverage.
