About the Job

Job Description

In this role, you will collaborate with the interdisciplinary team, patient/family/caregiver, and primary care provider to develop a holistic plan of care focused on achieving patient-centered goals. Your responsibilities will include:

You will also be responsible for maintaining communication with case managers and other disciplines via care conferences or other appropriate methods, reporting acute medical problems and condition changes to the primary care provider, and providing supervision to Physical Therapy Assistants, Home Health Aides, and students in the clinical setting.

At least two (2) years of physical therapy experience is highly preferred, as is one (1) year of experience in home health or other health settings with a geriatric emphasis. You must also possess a current license in Physical Therapy or be able to obtain prior to hire, as well as American Heart Association Healthcare Provider BLS certification upon hire and within 90 days of hire.
