Job Title: Occupational Therapist

Job Summary:

We are seeking a highly skilled and compassionate Occupational Therapist to join our team at Focus Health. As a Home Health Occupational Therapist, you will play a vital role in helping patients recover, improve, and maintain skills needed for daily living.

Responsibilities:
  • Evaluate patient functional status, including muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability, and work capacity.
  • Evaluate the home environment for hazards or barriers to more independent living.
  • Identify equipment needs and provide supervision to Occupational Therapy Assistants.
  • Conduct initial evaluations, assessments, and report findings to the Health Care Provider and RN, and develop plans of care promptly for patients.
  • Document initial evaluations, treatment plans, and re-evaluations of patient progress at least every thirty (30) days and update plans of care as necessary.
  • Complete comprehensive assessments and OASIS data, as indicated on a timely basis.
  • Observe, record, and report to the Health Care Provider the patient's treatment, response, and progress, and promptly report significant changes.
  • Obtain orders to revise the plan of care and instruct patients/CG in the use of alternate methods and devices for upgrading independence in activities of daily living.
Qualifications:
  • Bachelor's Degree and active Occupational Therapist license in the state of practice.
  • Work experience of at least one (1) year under a qualified Occupational Therapist, home health experience preferable.
  • Ability to make home visits and provide occupational therapy evaluations and treatments in the home environment, complete documentation effectively, and submit in a timely manner.
