Job Title: Home Health Registered Nurse

Job Summary:

We are seeking a skilled and compassionate Registered Nurse to provide comprehensive care to patients in their homes. The ideal candidate will have a strong background in nursing, excellent communication skills, and the ability to work independently in a fast-paced environment.

Responsibilities:
  • Provide skilled nursing visits to patients in their homes, assessing their needs and developing individualized care plans.
  • Confer with physicians and other healthcare professionals to ensure seamless care coordination and communication.
  • Administer medications, perform wound care, and provide education to patients and their families on disease management and self-care.
  • Collaborate with interdisciplinary teams to develop and implement comprehensive care plans that address the physical, emotional, and social needs of patients.
  • Document patient progress and outcomes, ensuring accurate and timely reporting to physicians and other stakeholders.
Requirements:
  • Diploma, Associate, or Bachelor's degree in Nursing.
  • Current and unrestricted Registered Nurse licensure.
  • Minimum of one year of nursing experience, preferably in home health or a related field.
  • Strong organizational and communication skills, with the ability to work effectively in a fast-paced environment.
