This full-time position offers flexible scheduling with the ability to balance work and personal life. With no on-call requirements, weekends, or holidays, you'll enjoy a healthy work-life balance.

You'll be responsible for conducting home visits across the state of Mississippi, approximately 85% of the time. All travel expenses are covered, and you'll receive a competitive base salary plus incentives.

Key Responsibilities:

  • Conduct thorough in-home assessments, including medical history evaluations, system reviews, medication reconciliations, vital sign checks, and comprehensive physical exams
  • Perform evidence-based practice screenings, including point-of-care testing (as necessary)
  • Determine diagnoses to inform care management and active medical management in treatment
  • Formulate lists of current and past medical conditions using clinical knowledge and judgment, along with assessment findings
  • Communicate assessment results to inform members' primary care physicians of potential care gaps
  • IDentify urgent and emergent situations and respond appropriately
  • Educate members on disease processes, medications, and compliance

Required Qualifications:

  • Hold an active NP license or obtain one by the start date; licensure must be unencumbered and free from any disciplinary actions, restrictions, or probation
  • Possess active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult-Gerontology Primary Care, or Adult-Gerontology Acute Care, or be able to obtain certification and/or NP licensure in the state of assignment by the start date
  • Meet the physical demands of the job, including lifting a 30-pound bag, navigating stairs, and performing physical assessments
  • Have access to reliable transportation for up to 85% of travel within licensed states (with notice, based on business needs) to complete home visit assessments
