About the Role

We're seeking a highly skilled clinician to join our interdisciplinary team as an Advanced Practitioner in skilled nursing and rehabilitation. In this role, you'll be responsible for delivering comprehensive care to patients, including assessing their needs, developing treatment plans, and collaborating with other healthcare professionals to ensure seamless transitions of care.

You'll work closely with our operations team to ensure that patients receive the highest level of care possible, and that our facilities are running smoothly and efficiently. This includes rounding on patients as medically necessary, conducting H&P's, admissions, progress notes, transfers, discharges, and acute/follow-up care. You'll also work with the facility to transfer patients to the most appropriate level and site of care upon discharge, whether that's home, independent living centers, assisted living, or memory care.
