As a SWAT RN (Specialized Workforce and Transport), you will play a crucial role in our healthcare team. Your primary workplace will be at Banner University Medical Center Tucson, with potential assignments at Banner University Medical Center South Tucson as necessary.

POSITION SUMMARY

This role involves assessing, planning, implementing, evaluating, and documenting nursing care for patients in line with organizational policies and professional nursing standards.

You will be accountable for the quality of nursing services provided by yourself and those under your supervision.

This position requires specialized knowledge, judgment, and nursing skills to assess data and plan, deliver, and evaluate care appropriate to the physical and developmental age of assigned patients.


CORE FUNCTIONS

  • Conducts comprehensive assessments of patients' physical, psychological, social, spiritual, educational, developmental, cultural, and discharge planning needs. Reviews patient history and physical information with patients and families, ensuring timely completion. Evaluates available information from other healthcare team members and reviews diagnostics and laboratory data, reporting any abnormalities to the physician(s) and relevant caregivers. Completes assessments and reassessments based on patient needs and policy guidelines.
  • Develops a care plan, including discharge planning, based on assessment data and input from patients, families, and the healthcare team. Initiates a patient-specific care plan that reflects realistic and measurable goals, considering the physical, cultural, psychosocial, spiritual, age-specific, and educational needs of the patient. Collaborates with multidisciplinary team members to review and adjust the care plan as necessary.
  • Implements the care plan through direct patient care, coordination, delegation, and supervision of healthcare team activities. Provides care according to physician orders and the nursing care plan, adhering to policies, procedures, and regulatory requirements. Delegates tasks appropriately and supervises nursing care provided by other licensed nurses and personnel. Ensures continuity of care by effectively communicating patient status to other caregivers.
  • Evaluates patient and family responses to care and education, adjusting the care plan as needed. Collaborates with healthcare team members to assess patient progress towards goals and expected outcomes. Reviews the effectiveness of patient education.
  • Documents all aspects of assessment, planning, implementation, and evaluation in the patient record, ensuring legibility, timeliness, and compliance with policy. Documentation should reflect objective and subjective data, nursing interventions, and patient responses to treatment. Accurately notes physician orders in a timely manner.
  • Provides care based on the best available evidence and may engage in research activities within clinical practice. Participates in unit or facility shared leadership and contributes to the education and development of facility personnel, patients, students, families, and visitors. Supports the growth of others through mentoring and precepting.
  • Contributes to excellent patient outcomes through timely, effective, efficient, equitable, and safe care. Actively participates in improving national nursing quality indicators and outcomes, which may include involvement in professional organizations.
  • In certain roles, this position may involve supervising staff and managing departmental workflow.

9. Engages with all levels of staff across various departments, physicians, patients, families, and external contacts regarding patient health care.

Collaborates with other healthcare providers to report and clarify information, and works with clergy to provide spiritual support.

Integrates and prioritizes data from multiple sources to support the human response of patients and families to changes in health status.


MINIMUM QUALIFICATIONS

Must hold a current, valid RN license in the state of practice, a temporary/interim RN license, or compact RN licensure for the current state of practice.

BLS certification is mandatory. Additional certifications or continuing education may be required based on the area of practice.

Experience in the clinical area for which you are applying is preferred. Must maintain clinical performance competencies relevant to the area of work as demonstrated through annual validations.

Positions within Banner Registry and Travel acute care require a minimum of one year of experience in an acute care hospital setting.

Experience must include working in an acute care environment within the past 12 months as a Registered Nurse in the specialty area.

Banner Registry and Travel physician practice positions require at least one year of experience as a Registered Nurse in a physician practice or acute care setting.


In a Home Health setting, team members must operate a personal vehicle and possess a valid driver's license.

Auto insurance coverage must be active and maintained.


Employees at Banner Behavioral Health Hospital, BTMC Behavioral, Banner Home Care/Hospice, or Boswell Skilled Nursing Facility must possess an Arizona Fingerprint Clearance Card at the time of hire and maintain it throughout their employment.

Employees at Banner MD Anderson on the Banner University Medical Center Phoenix campus must possess a State of Arizona Department of Public Safety Level One Fingerprint Clearance Card at the time of hire and maintain it throughout their employment.

This is a requirement for the Whole Family Counseling Program at this specific facility.

Employees in the Banner McKee Senior Behavioral Health Inpatient Unit must possess a Colorado Fingerprint Clearance Card at the time of hire and maintain it throughout their employment.

Employees in any Banner Staffing (BSS) Behavioral Health role in Arizona must possess a State of Arizona Department of Public Safety Level One Fingerprint Clearance Card at the time of hire and maintain it throughout their employment.


PREFERRED QUALIFICATIONS
A Bachelor's degree is preferred. Professional certification is also preferred.

Additional related education and/or experience is preferred.
