The Social Worker plays a vital role in identifying the social and emotional needs of each client. This position assists in arranging services to meet the needs of residents, providing liaison between the client, family, facility staff, and the community.
Key Responsibilities- Assess patients admitted to the Behavioral Health department and gather information from available sources.
- Formulate and communicate treatment information to the interdisciplinary treatment team.
- Participate in the development of the patient's treatment plan and coordinate or provide services.
- Serve as a resource to staff and community regarding psychosocial issues.
- Master's Degree in Social Work from a school approved by the Council on Social Work Education.
- Current licensure as a Licensed Social Worker (LSW) or a Licensed Clinical Social Worker (LCSW).
- Current American Heart Association CPR Certification.
- Proficient in Microsoft Office Software (Word and Excel).
- 2 years of hospital social work or relevant fieldwork placement in mental health or home care.
