Under the direction of a Licensed Masters Social Worker, this role provides behavioral health consultation and services to children, adolescents, adults, and families to improve psychosocial functioning. The successful candidate will consult with and make recommendations to the care team relative to psychological/substance use disorder (SUD) intervention strategies and conduct follow-up consultations to reinforce care team-generated interventions.
Key Responsibilities- Accept referrals from the care team regarding patients' behavioral health needs and respond in a timely manner.
- Provide comprehensive assessment, diagnosis, consultation, and interventions for behavioral health, and/or substance use needs, and/or social determinants of health needs.
- Assist the care team in developing care management processes such as the use of guidelines, disease management techniques, case management, and patient education to improve self-management of chronic conditions.
- Consult and collaborate with patients and the care team regarding recommended behavioral health interventions.
- Maintain pertinent, accurate, and timely documentation.
- Master's degree in social work or counseling from an accredited graduate school.
- Licensed as a Licensed or Limited Licensed Master's Social Worker or Licensed Professional Counselor in the State of Michigan.
- Basic Life Support (BLS) certification required.
- Preferred minimum of three (3) years of experience in a behavioral health setting.
