About the Role:

  • Deliver patient-centered care to individuals with complex needs.
  • Develop and implement effective care plans to promote positive patient outcomes.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Maintain accurate patient records and participate in quality improvement initiatives.

Requirements:

  • Active LPN licensure in Colorado and graduation from an accredited LPN program.
  • 6 months of experience as an LPN or CNA, with a focus on clinical skills and critical thinking.
  • CPR certification and valid TB screening.
  • COVID-19 vaccination card and proof of flu vaccination during flu season.

Estimated Salary:

The estimated annual salary for this position is approximately $65,500 - $70,000, based on a 36-hour workweek and assuming the top end of the hourly pay range ($35 per hour).
