Job Summary

We are seeking a highly skilled and dedicated Registered Nurse to join our team at Cobb and Douglas Public Health. As a Registered Nurse, you will play a vital role in promoting and protecting the health and safety of the residents of Cobb and Douglas counties.

Key Responsibilities
  • Assess clients with chronic medical conditions and develop individualized plans of care to optimize healthy outcomes.
  • Provide care coordination services and education to clients and their families, linking them to community resources and services.
  • Document all client encounters and maintain accurate medical records.
  • Consult and coordinate care with internal and external health care professionals to develop and evaluate comprehensive plans of care.
Requirements
  • Bachelor's degree in nursing from an accredited college or university.
  • One year of PHN experience and current Georgia license to practice as a Registered Professional Nurse.
  • CPR certification.
  • Bilingual English/Spanish.
  • Experience with case management, children and adolescents with chronic medical conditions and special healthcare needs.
