">Position Details
">- Location: Dublin, OH
- Contract Length: 13 weeks
- Schedule: Monday-Friday, 8 AM - 5 PM (possible weekend call rotation)
Job Responsibilities
">- Deliver high-quality nursing care to patients in their homes.
- Conduct comprehensive patient assessments and develop individualized care plans.
- Document patient information in HomeCare HomeBase (HCHB) software.
- Educate patients and their families on health management and care practices.
- Collaborate with interdisciplinary teams to ensure seamless patient care.
Requirements
">- Active RN license in Ohio.
- Minimum of 2 years of recent home health experience.
- Proficiency in OASIS documentation and HCHB software.
- Current BLS certification.
- Strong communication and organizational skills.
