NP Housecalls Specialist in Rapides and Evangeline Parishes

As a NP Housecalls Specialist in Rapides and Evangeline Parishes, you'll play a vital role in our mission to provide holistic care that addresses patients' physical, mental, and social needs.
Key Responsibilities:- Conduct in-home assessments, including medical history reviews, medication reconciliation, and comprehensive physical exams.
- Perform evidence-based practice screenings, including point-of-care testing as needed.
- Identify diagnoses and develop care management plans to improve patient outcomes.
- Communicate findings with patients' primary care physicians to ensure seamless care coordination.
- Educate patients on disease management, medication adherence, and healthy lifestyle choices.
- Active NP license or ability to obtain by start date, with unencumbered licensure.
- APCs must have approval to practice autonomously or without formal supervision, if applicable.
- Active ANCC or AANP national certification in Family, Geriatrics, Adult, or Adult-Gerontology Primary Care, or the ability to obtain certification by start date.
- Ability to complete physical requirements of the job, including lifting, navigating stairs, and sitting/standing/kneeling for physical assessments.
- Reliable transportation for home visit assessments.
- 1+ years of clinical experience in the highest level of education, clinical setting.
- May be requested to obtain additional licensure in other geographic areas.
- Experience in gerontology, cardiology, internal medicine, or endocrinology.
- Home health care or home visit experience.
- Ability to travel throughout the state, beyond your assigned region, up to 5% of the time, with advanced notice and potential for additional compensation incentives.
