Job Summary

Kaleida Health is seeking a skilled Infusion Therapy Nurse to join our team. As a key member of our Home Care team, you will be responsible for coordinating the total Plan of Care for IV patients in the Home Care setting.

Key Responsibilities
  • Assess, plan, implement, and evaluate the care of IV patients in the Home Care setting.
  • Provide leadership for the IV Health Care team by working cooperatively with other team members to maintain standards of professional nursing practice.
  • Collaborate with other healthcare professionals to ensure seamless care coordination.
  • Develop and implement individualized care plans to meet the unique needs of each patient.
  • Monitor patient progress and adjust care plans as needed.
Requirements
  • Associates Degree in Nursing required.
  • 1-5 years of relevant experience in medical/surgical nursing and infusion therapy.
  • Current licensure to practice as a Registered Nurse in NYS.
  • Valid NYS drivers license.
  • Excellent communication and organizational skills.
