POSITION TITLE: Family Physician

DEPARTMENT: Medical Services

REPORTS TO: Chief Health Officer

OVERVIEW: The individual will serve as a vital member of the healthcare team, delivering exceptional medical services to all patients while adhering to the highest standards of care specific to the specialty. The physician will also collaborate as part of the healthcare team within the facility.

REQUIREMENTS: The physician must possess a valid medical license to practice in Louisiana along with a current DEA license. Board eligibility or certification in a primary care specialty is required. CPR certification is highly recommended.

EDUCATION AND EXPERIENCE:

  • Medical degree from an accredited institution or a DO degree from an approved osteopathic school.
  • Completion of a residency program to achieve board eligibility; board certification is preferred.
  • Possession of a valid Louisiana medical license and DEA license.
  • Comprehensive understanding of medical practice principles relevant to the specialty.
  • Awareness of the diverse cultural and economic backgrounds of patients.
  • Equal emphasis on health maintenance and disease prevention as well as treatment and recovery.
  • Active participation in provider staff meetings, general staff meetings, and quality assurance activities.
  • Commitment to meet the standards set by the SWLA Center for Health Services and adhere to Health Provider By-Laws.
  • Proficiency in using Centricity/EMR and the ability to maintain accurate and timely medical records.
  • Compliance with NCQA, JCAHO, and SWLA Center for Health Services EMR requirements within specified time frames.
KEY RESPONSIBILITIES:
  1. Ensure patient confidentiality and uphold patient rights.
  2. Exhibit punctuality and maintain good attendance.
  3. Engage with patients to meet their expectations.
  4. Conduct patient examinations to identify diseases and establish preventive health measures.
  5. Maintain clinical tracking data to ensure continuity of care.
  6. Assist the Medical Director in planning, managing, and evaluating healthcare delivery.
  7. Provide ongoing care by obtaining hospital privileges and emergency room coverage.
  8. Actively engage in local, state, and national medical associations.
  9. Participate in quality assurance patient review committees.
  10. Document medical records using the POMR system.
  11. Gather relevant data for clinical decision-making.
  12. Keep medical records up to date.
  13. Develop and implement personal work plans.
  14. Conduct periodic assessments and reviews of subordinate performance.
  15. Promote health by advising patients on diet, hygiene, and disease prevention methods.
  16. Participate in community education, staff training, and committee meetings.
  17. Exercise judgment to prevent service delays and mitigate health risks.
  18. Contribute to the infection control program at the SWLA Center for Health Services.
  19. Assist in other departments or centers as needed.
  20. Oversee the on-site clinic laboratory.
  21. Perform additional duties as assigned within the scope of clinical training and experience.
  22. Collaborate with Nurse Practitioners within the department.
