About the Role

CenterWell Home Health is seeking a skilled LPN Patient Services Coordinator to join our team. As a key member of our care team, you will play a vital role in ensuring the seamless coordination of patient care.

Key Responsibilities
  • Schedule visits and communicate with field staff, patients, physicians, and other stakeholders to maintain proper care coordination and continuity of care.
  • Assist with day-to-day office and staff management, including managing schedules for all patients.
  • Initiate infection control forms as needed and send completed reports to HRD for upload in the worker console.
  • Provide backup support during busy times, including receiving calls from field staff and assisting with weekly case conferences.
  • Maintain the client hospitalization log, including entering coordination notes and sending electronic logs to all office, field, and sales staff.
  • Ensure staff are scheduled for skilled nurse/injection visits unless an aide supervisory visit is scheduled in conjunction with the injection visit.
  • Schedule discharge visits/OASIS collection or recert visits following case conferences when tasks appear on the action screen.
  • Schedule TIF OASIS collection visits and delete remaining schedules.
  • Perform initial reviews of weekly schedules for productivity/geographic issues and forward schedules to the Branch Director for approval prior to distribution to staff.
  • Receive lab reports, assess for normality, fax copies to doctors, make copies for Case Managers, and route to Medical Records Department (if clinical).
  • May be required to perform patient visits and/or participate in on-call rotations (if clinical).
