Job Summary

We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our team as a Medication Care Manager at Sunrise Senior Living. As a Medication Care Manager, you will play a vital role in ensuring the highest quality of care and services for our residents by administering medications and treatments in a safe and organized manner.

Responsibilities
  • Administer medications and treatments to residents in accordance with their individualized care plans.
  • Conduct daily medication reviews and updates to ensure accurate and timely medication administration.
  • Document all medication administration, including any refusals or missed doses.
  • Collaborate with the Resident Care Director (RCD) and Wellness Nurse to ensure seamless medication management.
  • Participate in the development and implementation of individualized care plans for residents.
  • Conduct regular audits of medication carts and treatment areas to ensure compliance with safety protocols.
  • Provide education and support to residents and their families on medication management and care.
Requirements
  • Current LPN licensure in the state of employment.
  • Previous experience working with seniors and in a medication management role.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
  • Strong problem-solving and decision-making skills.
  • Ability to maintain confidentiality and handle sensitive information.
