We're redefining the boundaries of care coordination by integrating services across the entire continuum. This role will play a pivotal part in supporting our members through telephonic and in-person interactions in patient settings and home environments.
Main Responsibilities:- Establish strong communication networks with internal teams, members, and primary caregivers to ensure seamless collaboration.
- Negotiate with multiple organizations to facilitate efficient benefits coordination.
- Closely review member status and provide guidance to achieve progress toward established goals.
- Provide administrative support for Primary Care Physicians (PCPs) and/or Advanced Practice Clinicians (APCs).
- A current, unrestricted Registered Nurse license in Oregon.
- At least 3 years of RN experience.
- A minimum of 1 year of case management experience.
- Familiarity with electronic medical records systems.
- Basic proficiency in Microsoft Office applications.
