SUMMARY OF ROLE:
The Substance Use Care Coordinator plays a vital role within the Community Health Team at Lamoille Health Partners. This position focuses on collaborating with patients and their families or caregivers to establish and achieve health objectives.
In partnership with the primary care provider, the care coordinator will develop a personalized care strategy aimed at supporting the patient in reaching their health aspirations.
Key Responsibilities:
1. Provide comprehensive care coordination and transitional care interventions.
2. Facilitate referrals to community resources and offer behavioral health brief interventions and counseling.
3. Conduct mental health and substance misuse screenings and assessments.
The role emphasizes building a supportive therapeutic relationship with patients, guiding them towards improved health through counseling, motivation, and education.
The ideal candidate will possess a solid understanding of substance use, the American Society of Addiction Medicine (ASAM) criteria, family dynamics, community resources, and the regulatory and legal frameworks that influence health outcomes.
This position is integral to the primary care team, delivering direct services during patient consultations and scheduled visits.
