Key Responsibilities- Conduct thorough patient needs assessments to identify individualized care plans and coordinate referrals to health, wellness, and post-acute care resources.
- Collaborate with interdisciplinary teams to progress patient care plans and discharge plans, promoting cost-effective care and minimizing fragmentation.
- Ensure compliance with federal and state regulations surrounding the discharge process, maintaining accurate documentation in the medical record.
- Utilize predictive analytic software to inform care decisions and optimize patient outcomes.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless transitions of care.
- Participate in departmental and system performance improvement initiatives, contributing to the development of innovative solutions and best practices.
- Current licensure in Virginia as a Registered Nurse.
- Three years of recent experience in a clinical healthcare setting with responsibilities reflecting direct management of patient care, including planning, coordination, and delivery of needed services.
- Supervisory or leadership experience is preferred.
- Strong knowledge and competency in areas such as computer literacy, community and system resources, effective interpersonal relations, assertiveness, flexibility, perseverance, diplomacy, and negotiation.
