Key Responsibilities- Develop and Implement Care Plans
- Collaborate with interdisciplinary teams to develop and implement comprehensive care plans for patients with complex wounds.
- Conduct thorough assessments of patients' physical and emotional needs, and develop individualized care plans to address these needs.
- Provide Expert Wound Care
- Provide high-quality, evidence-based wound care to patients, including debridement, dressing changes, and other treatments as needed.
- Monitor patients' wound progress and adjust care plans as necessary to ensure optimal healing outcomes.
- Collaborate with Interdisciplinary Teams
- Work closely with physicians, therapists, and other healthcare professionals to ensure seamless care coordination and optimal patient outcomes.
- Participate in interdisciplinary rounds and meetings to discuss patient care and develop strategies for improving patient outcomes.
- Education and Training
- Develop and implement educational programs for patients, families, and healthcare staff on wound care and related topics.
- Provide training and mentorship to new staff members on wound care best practices and protocols.
- Quality Improvement
- Participate in quality improvement initiatives to identify areas for improvement in wound care and develop strategies for improving patient outcomes.
- Contribute to the development and implementation of policies and procedures related to wound care.
- Education
- Bachelor's degree in Nursing required.
- Master's degree in Nursing preferred.
- Experience
- Minimum of five years of clinical experience in wound care or a related field.
- Experience in developing and implementing care plans and providing expert wound care.
- Certifications and Licenses
- Current registration to practice as a Registered Professional Nurse in the State of New York.
- Clinical certification in wound care or a related field preferred.
