Job Summary

We are seeking a highly skilled Social Work Case Manager to join our team at Yale New Haven Health. As a key member of our healthcare team, you will be responsible for ensuring high-quality patient care that is coordinated, efficient, and aligned with our institutional clinical and financial objectives.

Key Responsibilities
  • Coordinate Patient Care: Collaborate with the healthcare team to ensure the implementation of the plan of care, utilizing the principles of case management.
  • Optimize Hospital Systems: Identify and monitor compliance with documenting variances from established parameters in the clinical pathway or treatment plan.
  • Ensure Patient Needs are Met: Utilize information obtained from various resources to ensure that each patient meets the clinical needs for admission, treatment, and discharge.
  • Develop and Implement Discharge Plans: Ensure that an appropriate discharge plan is developed and implemented with the healthcare team members, including identifying service, treatment, and funding options.
  • Collaborate with Healthcare Teams: Work collaboratively with the healthcare team to actively involve clinical nurses in the assessment and planning for patient discharge to a facility.
  • Advocate for Patients: Act as a patient advocate, exhibiting awareness of ethical and legal issues concerning patient care and striving to manage situations to reduce risk.
Requirements
  • Education: Bachelor's Degree in Social Work (BSW) required, Master's Degree in Social Work (MSW) preferred.
  • Experience: Minimum of 2 years of relevant experience in case management or a related field.
  • Skills: Lead Transitional Care rounds with physicians, nurses, and other members of the healthcare field. Case management experience a plus.
