Health Unit Coordinator

Job Summary:
The Health Unit Coordinator (HUC)/Certified Nursing Assistant (CNA) is a combined role that functions under the supervision of physicians or registered nurses to provide coordination of admissions, transfers, discharges, and provides patient/family care plans based off of a collection of patient information.
Key Responsibilities:
- Coordinate patient admissions, transfers, and discharges
- Develop and implement patient/family care plans
- Communicate and collaborate with healthcare team members to facilitate patient care
- Provide patient care for significantly achievement of patient and family outcomes
- Facilitate the acquisition of materials and assist with clerical activities to support the units operations
- Complete routine and delegated direct patient care activities communicated by the registered nurse
Requirements:
- Must have or has had a Certified Nursing Assistant (CNA) certification awarded by the State of Wisconsin
- Basic Life Support (BLS) certification awarded by the American Heart Association (AHA) within three months of hire
- Experience working as a Health Unit Coordinator and a Certified Nursing Assistant (CNA) is preferred
