We are seeking a motivated and passionate Physical Therapist to join our team in a per diem capacity. As a Physical Therapist with Metro, you will have the opportunity to work with patients in their homes, providing personalized care and treatment plans to help them achieve their goals.

Key Responsibilities:

• Develop and implement individualized treatment plans for patients in their homes
• Conduct thorough assessments and evaluations to identify patient needs and goals
• Provide skilled therapy services to patients, including physical therapy, occupational therapy, and speech therapy
• Collaborate with patients, families, and healthcare teams to ensure seamless care coordination
• Document patient progress and outcomes in a timely and accurate manner

Requirements:

• Current Physical Therapist license in the state of New York
• Experience in home health or per diem settings preferred
• Ability to work independently and as part of a team
• Strong communication and interpersonal skills
• Proficiency in electronic medical records (EMRs)
