Key Responsibilities- Develop and implement comprehensive diabetes educational programs and activities to increase knowledge, skills, and competencies of individuals with diabetes.
- Collaborate with healthcare teams to ensure seamless care coordination and patient-centered approaches.
- Provide education and support to patients and their families to empower them to manage their diabetes effectively.
- Stay up-to-date with the latest diabetes management guidelines and best practices to ensure evidence-based care.
- Associate's Degree in Nursing from an accredited program.
- Current Alaska Registered Nurse License.
- Diabetes Educator certification or ability to achieve within a defined period.
- At least 2 years of patient care experience.
- Bachelor's Degree in Nursing or Nutrition.
- Case management experience.
