- You will collaborate closely with the multidisciplinary team to design and implement individualized treatment plans, incorporating assessment, individual, family, and group therapy sessions.
- Through meticulous record-keeping and documentation, you will track patient progress, update treatment plans, and communicate effectively with patients, families, and referring physicians.
- An essential aspect of your role involves coordinating treatment with the multidisciplinary team, patients' families, referral sources, and aftercare providers, facilitating smooth transitions and successful discharge planning.
- You will be responsible for maintaining accurate and up-to-date patient records, adhering to established departmental policies and procedures.
Shift Details: The primary work schedule includes night shifts (7p-7a), with regular every-other-weekend rotations.
