Job Description

The Adult Care Specialist - Behavioral Health will provide whole person care management services to adults with intellectual and developmental disabilities, mental health, and/or substance use needs. The specialist will work closely with integrated care teams, community organizations, and RHA departments to ensure the success of care management implementation.

The ideal candidate will have a master's degree in a human service field AND at least one year of full-time experience working with the MH/SU/IDD population or a bachelor's degree in a non-human service field AND at least four years of full-time experience working with the MH/SU/IDD population. They will also have two years of prior LTSS and/or HCBS coordination, care delivery monitoring, and care management experience.

Key Responsibilities
  • Provides whole person care management services to adults with intellectual and developmental disabilities, mental health, and/or substance use needs.
  • Works closely with integrated care teams, community organizations, and RHA departments to ensure the success of care management implementation.
  • Fosters and facilitates the care management process.
  • Conducts Care Management Comprehensive Assessments.
  • Shares information and facilitates Tailored Care Management Planning with the person supported and other team members.
