About the Role
We are seeking a highly skilled and compassionate Registered Nurse to join our team at Summit Health. As a Registered Nurse, you will be an integral part of a multidisciplinary care team, providing individualized, team-based care to our patients.
Key Responsibilities
- Assist physicians and staff with daily patient care coordination and access.
- Function as a liaison between patients, staff, and physicians.
- Maintain efficient patient flow.
- Deliver direct patient care within the scope of licensure and in accordance with SMG policy.
- Collect lab specimens and accurately label them.
- Perform Point of Care testing, EKGs, and phlebotomy as applicable.
- Prioritize patient care by implementing physician orders and nursing procedures.
- Ensure proper preparation for patient visits and prepare pertinent information.
- Manage nurse visit schedules and prepare necessary information.
- Perform patient intake, check-in/check-out as needed.
- Effectively communicate problems, concerns, or issues to the Office Supervisor/Manager.
- Assure all patients are correctly identified prior to providing and/or documenting care.
- Employ timely use of Tasking in EHR.
- Manage Clinical Nurse/Assigned inbox.
- Obtain vital signs and document them accurately in EHR.
- Administer injections according to protocol and document them in EHR.
- Perform patient outreach to satisfy quality measures.
- Coordinate and facilitate daily huddles.
- Demonstrate telephone triage protocols and document in EHR.
- Schedule appointments according to department guidelines.
- Identify and schedule annual visits as applicable.
- Monitor daily hospital discharge list and collaborate with Care Management.
- Adhere to universal safety precautions when administering medications and disposing of medical waste.
- Review and update medication lists in EHR.
- Initiate and follow through with prior authorization requests, DME orders, and home health referrals.
- Enter orders and schedule appointments in the Practice Management System and EHR.
- Provide referrals and/or consult orders in a timely manner.
- Check and maintain logs for emergency equipment.
- Facilitate transition of care to UCC/Hospital.
- Maintain proficiency with department-specific equipment and report malfunctions promptly.
- Participate in pilot programs.
Requirements
- Associates degree required; Bachelor's degree preferred.
- New Jersey State Nursing License and BLS certification required.
- 0-1 years of experience; 2-4 years preferred.
- Ability to communicate in English, both orally and in writing.
- Strong interpersonal and organizational skills.
- Ability to perform diverse work assignments with time limitations with a high degree of accuracy.
- Ability to use problem-solving, critical thinking, and priority-setting skills.
- Experience with Standard Office Equipment and Technology preferred.
