We are seeking a skilled Speech Language Pathologist to join our Home Health team on a PRN basis. As a Speech Language Pathologist, you will play a vital role in evaluating, directing, and providing speech/language pathology services to patients in their homes or facilities.
Key Responsibilities:- Evaluate patients' speech and language abilities to identify areas of need and develop personalized treatment plans.
- Direct and provide speech/language pathology services to patients, including assessment, diagnosis, and treatment of communication disorders.
- Collaborate with healthcare teams to ensure comprehensive care and effective communication with patients and families.
- Participate in care coordination activities and discharge planning to ensure seamless transitions and optimal patient outcomes.
- Document patient progress and maintain accurate records to support quality care and compliance.
- Meet the education and experience requirements for Certification of Clinical Competence in Speech Language Pathology or Audiology granted by ASHA.
- Minimum of six months experience as a speech therapist/speech language pathologist.
- Home Health experience a plus.
- Current and unrestricted license.
- Current CPR certification.
- Good organizational and communication skills.
- A valid driver's license, auto insurance, and reliable transportation are required.
