We are seeking a highly skilled Pharmacist to join our team at Intermountain Healthcare. As a Clinical Pharmacist, you will play a critical role in providing exceptional patient care and contributing to the development of our pharmacy services.
Key Responsibilities- Provide clinical pharmacy services to patients, including medication therapy management and disease state management.
- Collaborate with healthcare teams to develop and implement patient care plans.
- Conduct medication use evaluations and make recommendations for improvement.
- Participate in quality improvement initiatives and contribute to the development of pharmacy policies and procedures.
- Stay up-to-date with the latest clinical guidelines and research in pharmacy practice.
- Doctor of Pharmacy (PharmD) degree from an accredited institution.
- Current licensure as a pharmacist in the state of Utah.
- Board certification in a relevant specialty (e.g. pharmacotherapy, critical care pharmacy).
- Excellent communication and interpersonal skills.
- Ability to work effectively in a team environment.
