About the Role:
The Community Support Specialist in Assertive Treatment Services plays a crucial role in delivering intensive, community-based mental health support. This position is designed to empower individuals experiencing severe mental health challenges to achieve independence and integrate into their communities while pursuing their recovery objectives.

Key Responsibilities:
1. Conduct thorough assessments to develop personalized care plans addressing individual needs and desired outcomes.
2. Assist clients in navigating and securing appropriate medical coverage to ensure continuous access to comprehensive healthcare services.
3. Engage in outreach, advocacy, and coordination of resources, linking clients to both internal and external support systems.
4. Provide community-based assistance in diverse environments, including homes and public spaces, to facilitate treatment goals.
5. Manage crises effectively and coordinate necessary medical and emergency interventions.
6. Collaborate with fellow team members, natural supports, and external providers to ensure cohesive service delivery aligned with the Assertive Community Treatment model.
7. Foster individual recovery, resilience, and essential life skills to promote independent living.
8. Participate in on-call duties, including after-hours and weekend coverage on a rotating schedule.
9. Contribute to relevant committees as directed by supervisors.
10. Uphold professional standards, ensuring the confidentiality and rights of all clients are respected.
11. Complete all required documentation in accordance with agency policies and applicable regulations.
12. Maintain professional credentials and memberships.
13. Represent CLM with professionalism in all interactions.
14. Perform additional duties as assigned by supervisors.

Qualifications:
- A Bachelor's degree is preferred; however, an Associate's degree combined with a minimum of two years of relevant experience in the mental health field may be acceptable.
- Experience with co-occurring disorders is advantageous.
- Strong clinical judgment skills in various clinical situations.
- Proficiency in Microsoft Office applications and electronic record-keeping.
- Excellent documentation abilities and organizational skills.
- Effective communication skills and the capacity to manage multiple tasks.

Physical Requirements:
The role involves significant community engagement, including home visits. Candidates must possess reliable transportation and maintain a clean driving record.
