The Dialysis Registered Dietitian plays a crucial role within the interdisciplinary team, utilizing nutritional education to enhance the quality of life for our patients.
Key Responsibilities:
- Assist patients in making informed dietary and nutritional choices that positively impact their quality of life.
- Conduct thorough nutrition assessments for all new and at-risk patients.
- Collaborate with the interdisciplinary team and patients to create and implement tailored care plans based on nutritional evaluations; review and document progress monthly.
- Identify specific interventions necessary to meet nutritional needs, guided by evidence-based practices and current research.
- Educate patients and their families about renal diet and fluid management.
- Monitor monthly lab results and weight changes, providing recommendations to achieve established goals.
- Employ a patient-centered care approach, respecting individual and cultural preferences while fostering engagement.
- Participate in the Quality Assessment Performance Improvement process, documenting and attending meetings as per clinic policy.
- Develop and compile nutrition materials to encourage adherence to renal diet guidelines.
- Communicate nutrition-related concerns with the care team, including attending physicians.
- Share advancements in kidney disease and nutrition with the care team as necessary.
Qualifications:
- Excellent communication skills.
- Proven clinical proficiency.
- A collaborative spirit with care teams.
- Strong problem-solving abilities.
Education and Training:
- Must be a Registered Dietitian Nutritionist with the Commission on Dietetic Registration.
- A minimum of one year of professional experience in clinical nutrition as a registered dietitian is required.
- Previous experience in renal care is preferred but not mandatory.
