- Develop and implement individualized care plans, taking into account patients' unique needs and circumstances.
- Collaborate with healthcare professionals to ensure seamless transitions of care and effective communication.
- Provide education and support to patients and their families, empowering them to take an active role in their care.
- Participate in quality improvement initiatives, contributing to the development and implementation of best practices in dialysis care.
- Current Registered Nurse (RN) license in the state of practice.
- Associates Degree in Nursing (A.D.N) from an accredited school of nursing required; Bachelor of Science in Nursing (B.S.N) or three-year diploma from an accredited diploma program may be substituted for nursing degree.
- Experience in intensive care unit (ICU), critical care unit (CCU), emergency room (ER), or medical surgical (Med Surg) settings preferred.
- Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN) certification a plus.
