Physical Therapist Outpatient Specialist

We are seeking a highly skilled Physical Therapist to join our team at Yale New Haven Health. As a Physical Therapist, you will be responsible for evaluating and treating patients in a variety of inpatient and outpatient settings. You will work collaboratively with an interdisciplinary treatment team to provide comprehensive care to patients.
Key Responsibilities- Evaluate and treat patients with a range of diagnoses, including post-surgical, developmental, neurological, genetic, oncologic, and traumatic conditions.
- Develop and implement individualized treatment plans to meet the unique needs of each patient.
- Collaborate with other healthcare professionals to ensure seamless care coordination and communication.
- Provide education and training to patients and their families on proper exercise techniques, injury prevention, and disease management.
- Conduct thorough assessments and evaluations to identify patient needs and develop effective treatment plans.
- Document patient progress and outcomes in a clear and concise manner.
- Stay up-to-date with the latest research and best practices in physical therapy to provide evidence-based care.
- Graduate of a CAPTE-accredited program in Physical Therapy or equivalent.
- Current licensure or temporary permit to practice as a Physical Therapist in Connecticut.
- Basic Life Support (BLS) certification or ability to obtain within two months of hire.
- Ability to work in a fast-paced environment and prioritize multiple tasks and responsibilities.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Experience working with children and/or adults in a variety of settings, including inpatient and outpatient environments.
- Specialized training or certification in areas such as neurology, orthopedics, or pelvic health.
- Ability to work with a diverse patient population, including those with complex medical conditions.
