Full-Time Skilled Nursing Care Coordinator

We are seeking a compassionate and motivated Home Health Registered Nurse to join our team at 219 Health Network in Hobart, IN. As a key member of our multidisciplinary team, you will have the opportunity to make a meaningful difference in the lives of our patients.
About the PositionThis full-time role offers a competitive salary of around $70,000 - $90,000 per year, based on experience, as well as a comprehensive benefits package and opportunities for professional growth and development.
Duties and Responsibilities- Evaluate and assess patients' physical and psychological needs, developing personalized care plans to promote optimal outcomes
- Work collaboratively with patients, families, physicians, and healthcare teams to ensure seamless care coordination and delivery
- Foster open communication and trust with patients and families, addressing concerns and providing education on self-care techniques and disease management
- Bachelor's degree in Nursing (BSN) or related field
- Valid Indiana RN license
- Current CPR certification and relevant skills training
- At least one year of experience in acute care or long-term care nursing settings
