Key Responsibilities:
- Provide skilled nursing care to patients in their homes, promoting independence and well-being.
- Conduct thorough assessments and develop individualized care plans to meet patients' unique needs.
- Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
- Manage assigned caseloads, conducting 6-8 visit points during designated field days, including initial and reassessment visits.
- Demonstrate competency with OASIS tools and lead interdisciplinary teams as needed.
Requirements:
- Current licensure as a Registered Professional Nurse in New York State.
- Recent community health care, acute care, or long-term care experience.
- Minimum 2 years of experience as a Registered Professional Nurse in medical, surgical, or emergency room settings.
- Two to four years of home care experience preferred, with strong clinical skills.
- Knowledge of Medicare and Medicaid regulations.
- A valid NYS driver's license and reliable transportation for home visits.
- Physical and mental ability to perform job responsibilities.
- Strong interpersonal, problem-solving, and communication skills.
