Physical Therapist

BAYADA Home Health Care is seeking a skilled Physical Therapist to join our team for per diem opportunities performing home health visits. This role involves working with adult and geriatric clients in Cleveland and Gaston counties, providing rehabilitative care and therapy to enable them to receive medical care at home.
Responsibilities- Make home visits to clients in designated territories to assess and address their physical therapy needs.
- Perform diagnostic tests and measurements to evaluate clients' mobility, strength, and balance.
- Develop and implement individualized care plans, including manual therapeutic exercises, gait training, and other interventions.
- Educate clients and their families on rehabilitative care and activities to promote independent living.
- Accurately document observations, interventions, and evaluations using a state-of-the-art touch pad tablet.
- A current NC Physical Therapist license.
- A minimum of one year of recent work experience as a Physical Therapist.
- Graduation from a program approved by the American Physical Therapy Association or the Committee on Allied Health Education and Accreditation of the American Medical Association.
- Ability to work independently and manage time effectively.
- Strong interpersonal, organizational, and problem-solving skills.
- Solid computer skills, with prior experience with electronic medical records (EMR) preferred.
