Position Overview
We are looking for a skilled physical therapist to join our rehabilitation team at Fairview Health Services. In collaboration with patients and their families, the physical therapist will ensure the safe and effective delivery of patient care within their professional scope. This role encompasses examination, evaluation, diagnosis, planning, intervention, and outcome establishment.
- FTE 0.6, authorized to work 48 hours per pay period.

Fairview Health Services Rehabilitation provides a wide array of services catering to patients across multiple acute-care hospitals, post-acute settings, and outpatient therapy clinics. Our team includes specialists in Physical, Occupational, and Speech Therapy, as well as Audiology and Cardiac & Pulmonary Rehabilitation, collaborating with colleagues in various medical environments to offer numerous specialty programs.
As part of an academic health system with residency and fellowship programs, we emphasize a collaborative culture focused on learning and evidence-based, patient-centered care. Our continuing education program offers over 40 courses annually at no cost to employees, alongside support for numerous external continuing education opportunities.
As a member of the Fairview rehabilitation team, you will have access to formal mentorship, clinical specialization, and additional recognition and compensation through our clinical specialist program. Our nationally recognized programs position our therapists as essential contributors to helping patients achieve their goals and enhance their quality of life.
Key Responsibilities
The Physical Therapist collaborates with patients and their families during evaluations and re-evaluations. Responsibilities include:
- Collecting and applying relevant data, conducting examinations, and determining treatment diagnoses.
- Creating individualized care plans through effective coordination.
- Analyzing data to identify patient needs and potential risks, establishing recovery prognoses, and setting personalized goals.
- Implementing and adjusting care plans based on ongoing assessments and patient responses.
- Coordinating care and effectively communicating with the interdisciplinary team to ensure comprehensive care and appropriate discharge planning.
Patient Care and Intervention
Providing skilled interventions by:
- Modifying treatment plans to reflect changes in patient status and responses.
- Assessing learning needs of patients and/or caregivers and delivering appropriate education.
Documentation and Compliance
Maintaining professional standards by:
- Utilizing accepted medical terminology and adhering to professional practice standards.
- Completing documentation promptly in accordance with departmental guidelines.
- Ensuring compliance with departmental billing standards.
Professional Development
Fostering professional growth by:
- Prioritizing tasks and patient care for effective time management.
- Understanding and adhering to professional licensure and certification requirements.
- Providing guidance to team members and supervising students as necessary.
- Pursuing ongoing professional development to enhance clinical standards.
Quality Improvement Initiatives
Engaging in quality improvement efforts by:
- Developing and implementing enhancements, and participating in entity/system initiatives.
Qualifications
Required License/Certification/Registration
Physical Therapy registration/licensure in the state of Minnesota or a temporary license in MN.
Current Basic Life Support (BLS) certification.
Preferred Education & Experience
Doctorate degree in Physical Therapy.
Minimum of 1 year of practical experience in Physical Therapy.
