We are seeking a highly skilled Licensed Practical Nurse to join our Faculty Group Practice team at NYU Langone Health. As a key member of our team, you will provide exceptional patient care and support to our patients and families.
Responsibilities- Assist the RN in the care of patients and families by completing assigned patient care tasks, in accordance with the NYS LPN scope of practice and NYULH policy.
- Provide competent, safe, and compassionate care with emphasis on an understanding of the challenges faced by health maintenance, illness, hospitalization, undergoing procedures, or diagnostic testing.
- Check vital signs, record a patient's medical history, and utilize specialized skills and safety when carrying out specific tasks of patient care as assigned, including phlebotomy, point of care testing via finger stick, with certification, and electrocardiograms.
- Participate in patient-centered quality and performance improvement programs consistent with service and departmental programs.
- Acquire and maintain current LPN knowledge and competence to fulfill responsibilities.
- Support the missions, philosophy, standards, goals, and objectives of NYULH.
- Collect patient healthcare data systematically and, on an ongoing basis, using appropriate assessment techniques and instruments.
- Enforce adherence of NYULH and FGP policies and procedures and standards, including but not limited to HIPAA, Compliance, Human Resources, etc.
- Participate in coordination of unit activities to provide patient care in a healing, humane, and caring environment.
- Engage in self-assessment and participate in activities to promote own professional growth and development.
- Obtain patient vital signs, weights, EKGs as needed. Perform blood collection/phlebotomy as needed.
- Administer medications, including injections in accordance with NYULMC clinical guidelines and NYS Scope of Practice.
- Assist with maintaining patient flow to and from exam rooms and treatment areas. Assist in escorting patients. Ensure chart is available, with relevant lab work and test reports if indicated, for physician when the physician sees the patient.
- Maintain an awareness of current patient-centered evidence-based practices and use knowledge of applicable research data in provision of patient care.
- Collaborate with members of the interdisciplinary team and promote collegial relationships.
- Assist patients and communicate with licensed providers to obtain prescription renewals as needed.
- Perform patient-related functions as directed by the Licensed Provider and/or Registered Nurse (RN), and document properly. These may include the following, as governed by LPN scope of practice: medication administration, vaccination, other injections, and patch placement, set up and/or assist for procedure.
- Communicate directly with patients regarding their care or treatment using appropriate terminology and respectful manner. Interact with medical staff efficiently. Interact with support staff professionally.
- Serve as a support and resource for nursing staff and providers.
- Demonstrate knowledge of the NYUMC Service Standards and incorporate into the performance of responsibilities.
- Function as a respectful member of the health care team.
- Ensure clinical compliance with Patient Safety initiatives and reporting.
- Ensure clinical in-baskets are maintained and encounters are closed within a timely manner.
- Consider needs and behaviors of specific patient age and cultural groups in all patient care activities.
- Complete annual competencies such as Fire Safety, Hazardous Waste, Medsled, HIPPA compliance, etc.
- Serve as NYU Langone Health Faculty Group Practice Brand Ambassador by upholding the NYULH Mission, vision, and values and promoting excellence in the patient experience, during every encounter.
- Drive consistency in every patient and colleague encounter by embodying the core principles of our FGP Service Strategy CARES (Connect, Align, Respond, Ensure, and Sign-Off).
- Greet patients warmly and professionally, stating name and role, and clearly communicate each step of the care/interaction as appropriate.
- Work collaboratively with colleagues and site management to ensure a positive experience and timely resolution for all patient interactions and inquiries whether in person, by phone or via electronic messaging.
- Proactively anticipate patient needs, and participate in service recovery by applying the LEARN model (Listen, Empathize, Apologize, Resolve, Notify), and escalate to leadership as appropriate.
- Share ideas or any observed areas of opportunity, to improve patient experience and patient access, with appropriate leadership.
- Partner with Patient Access Center and Central Billing Office team members to support collaboration and promote a positive patient experience.
- Take a proactive approach in ensuring that practice staff are fully versed in the Access Agreement gold standard principles.
- Graduate of LPN program with current NYS OP LPN licensure and registration.
- Ability to complete multiple tasks efficiently and thrive in a team work environment which pursues a positive patient care experience.
- Physical stamina, manual dexterity, visual and aural acuity to perform responsibilities needed also required.
- BLS certification preferred.
