Salary Range:
">$65,000 - $85,000 per year, depending on experience.
Key Responsibilities:

- Conduct thorough assessments of patient needs, identifying opportunities for intervention and coordination with healthcare teams.">
- Develop personalized care plans, incorporating evidence-based guidelines and fostering collaboration with patients, families, and healthcare providers.">
- Ensure timely access to necessary services, including pre-authorizations, hospital reviews, and discharge planning.">
- Cultivate strong relationships with healthcare teams, promoting open communication and effective problem-solving.">
- Prioritize assignments, maintaining flexibility as new priorities arise and managing multiple tasks simultaneously.">
- Identify potential quality issues, variances, and never events, referring them to the Quality Improvement Department for further action.">
- Communicate effectively with patients, families, and healthcare providers, ensuring seamless transitions and optimizing health outcomes.">
- Foster a culture of continuous improvement, participating in quality improvement activities and contributing to the development of new programs and projects.">
Requirements:

- Active Ohio and WV licensure, with compliance with State Boards of Nursing continuing education policy.">
- Five (5) years of experience as a Registered Nurse, with two (2) years in a clinical setting.">
- Excellent oral, written, telephonic, and interpersonal skills, with the ability to balance independent and team working environments.">
- Flexibility and adaptability, with the capacity to work in fast-paced environments and adjust to changing processes.">
- Proficient keyboarding skills and computer literacy, with the ability to navigate multiple systems.">
- A commitment to Excellence and Accountability, with a focus on delivering high-quality clinical services.">
- Managed care experience, utilization management experience, or case management experience with a payor organization.">
- Certification in an area of clinical expertise related to current work.">
