Job SummaryWe are seeking a skilled Nurse Practitioner to join our team on a PRN/Part Time basis. As a Nurse Practitioner, you will conduct adult and geriatric assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, and follow-up.
Responsibilities- Conduct adult and geriatric assessments, medical history, diagnosis, and treatment, health education, physician referrals, case management referrals, and follow-up.
- Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
- Collaborate with Primary Care Physicians (PCPs) on patient education and provide follow-up.
- Provide services in a variety of venues, including home visits, skilled facility visits, and telehealth visits.
- Master's Degree required or commensurate experience and satisfactory completion of NP licensure.
- Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain prescriptive authority.
- Board certified by the AANP, ANCC, or AACCN in a Matrix-approved specialty.
- Current BLS, ACLS, or CPR certification.
- Minimum of 1 year experience as a Nurse Practitioner.
- Strong computer skills and familiarity with Employee Health/Medical Record software.
- Excellent verbal and written communication skills with patients, clients, and colleagues.
- Comfortable and flexible with frequent change.
- Ability to travel, valid state driver's license, and ability to drive a car, proof of adequate automobile insurance coverage for the state of residence.
- Travel required to meet patients where they live.
