Licensed Vocational Nurse

St. Joseph's Home Health, Inc. is seeking a skilled and compassionate Licensed Vocational Nurse (LVN) to join our team. As an LVN, you will play a vital role in providing high-quality patient care and support to our clients in the comfort of their own homes.

• Provide skilled nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
• Conduct thorough assessments of patients' physical and emotional needs, and develop individualized care plans to meet those needs.
• Collaborate with other healthcare professionals, including physicians and therapists, to ensure comprehensive care.
• Document patient progress and communicate with healthcare teams to ensure continuity of care.
• Provide education and support to patients and their families on disease management, medication use, and other health-related topics.

• Graduate of an accredited school of vocational nursing.
• Current LVN license in good standing.
• Proof of current CPR certification.
• Ability to work flexible schedule, including evenings and weekends.
• Minimum 1 year experience as an LVN in a clinical setting preferred.
