Senior Nurse Care Coordinator

We are seeking an experienced Registered Nurse (RN) to join our Center Care Team. This role involves direct patient care, education, and collaboration with providers and clinical operations staff.
About the Role
- This position reports to the Center Manager and requires daily responsibility in direct patient care and education.
- The ideal candidate has a passion for working with seniors, enjoys performing daily patient care, and desires a Monday-to-Friday schedule.
Responsibilities
- Perform daily nursing activities, including phone and in-person patient triage and patient education.
- Collaborate with Providers and Center Manager to identify opportunities for improving clinical performance and develop strategies to address them.
- Maintain current knowledge of state regulations for nursing, medicine, and pharmacy as it relates to scope of practice.
- Identify and provide resources for educating patients, their families, and staff members on clinical conditions and processes.
- Oversee clinical quality and patient safety through training and education of back-office personnel and administrative staff.
- Ensure adherence to clinical processes, policies, and procedures.
Requirements
- A valid, active Registered Nurse (RN) license in state(s) of employment required.
- A minimum of 5 years' direct nursing/medical work experience required.
- Work in a community-based clinic, public health department, or specialization with the senior population preferred.
Compensation
We offer a competitive salary range of $90,000 - $110,000 per year, depending on experience.
