As a Home Health RN, you will be responsible for providing high-quality nursing care to patients in their homes, working closely with other healthcare professionals to develop and implement individualized care plans.

Key Responsibilities:
  • Assess patients' physical and emotional needs, developing and implementing plans of care in collaboration with physicians and other healthcare professionals.
  • Provide skilled nursing visits and assessments, documenting patient progress and communicating with healthcare teams as needed.
  • Collaborate with Physical Therapist, Speech Therapist, and Occupational Therapist to ensure comprehensive care.
  • Participate in education programs, quality assessment activities, and staff meetings to stay up-to-date on best practices and company initiatives.
Requirements:
  • Active Registered Nurse or Licensed Practical Nursing license in the state of MO or Compact.
  • Home Health experience required.
  • Strong organizational and self-management skills, excellent communication skills.
