Key Responsibilities- Care Planning: Conceive and create comprehensive care plans based on thorough resident assessments, incorporating input from residents, families, and the care team.
- Assessment Coordination: Schedule and lead the assessment coordination process to ensure timely and accurate completion of MDS assessments.
- Data Management: Compile data for resident insurance updates and communicate effectively with the Director of Therapy and Insurance Case Managers to optimize reimbursement.
- Leadership: Lead the MDS completion process, ensuring accurate and timely completion of all sections by responsible disciplines.
- Licensure: Current RN licensure in the Commonwealth of Pennsylvania.
- Experience: Three years of experience in a similar role, with two years in Long Term Care.
- Certification: RAC-CTA certification preferred but not required.
- Skills: Strong communication and leadership skills, with the ability to coordinate and organize work to meet multiple job responsibilities and deadlines.
- Technical Skills: Proficient computer skills, with experience in Microsoft Office.
