About the Role

We are seeking a highly skilled and motivated Pharmacist to join our team at PharMerica. As a key member of our pharmacy team, you will play a critical role in providing exceptional patient care and support to our patients in long-term care settings.

Responsibilities
  • Evaluate physician medication orders for appropriateness, making recommendations for therapy changes as needed.
  • Maintain accurate and up-to-date patient/resident databases, including current and past medical history, clinical and laboratory data, and concurrent medication.
  • Collaborate with the healthcare team to provide clinical and technical support, ensuring the safe procurement, storage, distribution, and use of drugs and biologicals.
  • Document all communication with physicians, nurses, and other healthcare team members in the medical record.
  • Implement policies and procedures for the safe handling and disposal of medications.
Requirements
  • Bachelor's Degree in Pharmacy; Pharm D preferred.
  • One year experience in long-term care.
  • Licenses/Certifications: As required by government regulations and business needs.
