Job Overview: D&G Medical Associates Pllc seeks an experienced Care Coordinator to provide Chronic Care Management and Remote Patient Monitoring services to patients with complex medical conditions. The successful candidate will possess certification as a Nursing Assistant (CNA), Medical Assistant (MA), or MedTech, and have strong organizational skills with attention to detail.

Key Responsibilities:
  1. Patient Care Management: works diligently on behalf of assigned patients, ensuring their needs are met with empathy and efficiency.
  2. Requirements: possesses certification as a Nursing Assistant (CNA), Medical Assistant (MA), or MedTech, demonstrates passion for patient care, maintains a HIPAA-compliant workspace.
  3. Administrative Duties: meets or exceeds outbound connect rates, compliance standards, caseload management, billable time requirements.
  4. Coordination and Assistance: assists with delivery of Remote Patient Monitoring devices, handles pre-authorizations, referrals, prescription refill issues, and medication reconciliation tasks.
  5. Education and Support: provides patient education on prescriptions, disease processes, and lifestyle recommendations.
