General Overview:
The Long Term Care Nurse is responsible for the comprehensive delivery of care to residents within the Long Term Care sector. This role requires the nurse to demonstrate clinical judgment and critical thinking skills while coordinating resident care effectively.

Key Responsibilities:
Essential duties include but are not limited to:
  • Employing the nursing process to assess, plan, implement, and evaluate care for residents in the Long Term Care setting.
  • Conducting thorough assessments to identify the specific needs of residents.
  • Demonstrating proficiency in geriatric assessments and treatments.
  • Recognizing signs of abuse and neglect, and understanding the reporting procedures.
  • Evaluating safety concerns pertinent to the geriatric population.
  • Collecting and analyzing data to inform nursing diagnoses and care plans.
  • Documenting care plans with identified nursing diagnoses and expected outcomes.
  • Prioritizing actions based on resident care requirements.
  • Ensuring alignment between care plans and medical staff directives.
  • Providing a safe and therapeutic environment for residents.
  • Initiating and assessing educational programs for residents and their families.
  • Adjusting care plans based on evaluations and changes in medical directives.
  • Monitoring the effectiveness of interventions and documenting resident responses.
  • Engaging in effective communication and teamwork within the organization.

Qualifications:
  • Graduate of an accredited nursing program with valid licensure as an RN or LPN.
  • Certification in Basic Life Support from the American Heart Association is required.

Experience:
  • A minimum of one year of professional nursing experience is preferred.
  • Strong communication skills and the ability to work collaboratively in a team environment.

Physical and Mental Requirements:
  • Ability to lift and transfer residents safely, utilizing proper techniques.
  • Capacity to manage the physical demands of the role, including pushing and pulling equipment.
  • Adequate visual and auditory senses to provide effective resident care.
  • Ability to maintain focus and concentration in a dynamic environment.

Work Environment:
  • Potential exposure to infectious materials and diseases.
  • The work environment typically has a moderate noise level.
