Key Responsibilities
  • Provide hands-on advanced care in accordance with medical orders, including tilt-table therapy, bowel programs, medication management, and wound care.
  • Work collaboratively with our care team to develop and implement personalized care plans that meet the unique needs of each patient.
  • Conduct thorough assessments and evaluations to identify patient needs and develop effective care strategies.
  • Collaborate with healthcare professionals to ensure seamless care transitions and optimal patient outcomes.
  • Document patient progress and communicate effectively with the care team to ensure continuity of care.
Requirements
  • Licensed Practical Nurse (LPN) license in good standing.
  • 6 months of recent, relevant experience in home care or facility staffing.
  • Valid driver's license and auto insurance (if applicable).
  • Updated TB test and willingness to undergo COVID-19 testing as required.
  • Ability to pass a background check.
