  1. Conduct thorough assessments of patients' physical abilities and develop personalized plans of care to improve or maintain their functional capacity.
  2. Provide skilled physical therapy interventions, including exercises, stretches, and modalities, to help patients achieve their goals.
  3. Collaborate with other healthcare professionals, including physicians, nurses, and social workers, to ensure comprehensive care and optimal patient outcomes.
  4. Document patient progress and communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  5. Participate in quality improvement initiatives and contribute to the development of policies and procedures to enhance patient care and safety.
Requirements:
  1. Graduate from an accredited physical therapy program.
  2. Current licensure in the state(s) where practicing.
  3. Minimum 2 years of experience in home health physical therapy, preferred.
  4. Ability to work independently and as part of a team.
